What is a Release of Information or ROI?

A release of information, or ROI, is a form commonly used by adoptive parents when they are requesting an agency, hospital, school, or similar entity share confidential information with their attorney. Often times, the attorney will need certain information from the agency in order to complete the adoption process. The agency, in following their own best practices will require the adoptive parents to execute, or sign, a release of information.

The attorney may also need medical records from the hospital. The hospital may feel more comfortable releasing those HIPPA protected documents if the hospital receives a release of information from the adoptive family.

Follow this link to see a template release of information. It may be prudent for the adoptive family to carry several blank ROI forms with them, being ready to execute it when they encounter an entity that requires one.
